Billing

Your insurance company will be billed with the information that you provide to us for the services rendered at the time of your visit. We participate with most local and national insurers, HMOs, PPOs, IPAs and Workers Compensation carriers.

Please contact the MCAOS billing department @ 860 346-2267  to see if we participate with your insurance carrier. You are responsible for any deductibles,or co-pays. Co-pays are due at the time of your visit. it is important to inform your insurance company of your pending surgical procedure.

You will receive a bill from the Middlesex Center for Advanced Orthopedic Surgery for any balance that is billable to you after your insurance has paid. You will also receive a separate bill from your surgeon and anesthesiologist depending on which services were utilized when you were a patient at our center.

Workers Compensation

Worker's Compensation claims need additional information. Please make sure that you provide the following information prior to your surgical procedure:

• Worker's Compensation Insurance Carrier, representative's name and telephone number.
• Employer name and address
• Date of injury
• Claim number
