DO NOT EAT OR DRINK ANYTHING INCLUDING CHEWING GUM, CANDY OR WATER UNLESS INSTRUCTED BY OUR PROFESSIONAL STAFF. You may brush your teeth.

- Please do not wear contact lenses, make-up, nail polish or jewelry.
- Leave all your valuables at home, including money and wallet.
- Please remember to bring your ID and insurance cards and completed claim forms.
- Do not take your prescription medication on the morning of surgery unless instructed to do so by our professional staff.
Special Circumstances
Patients under the age of 18 years old MUST have a parent or legal guardian accompany them to the Surgery Center and remain at the Surgery Center until the patient is discharged. Parental consent is required for minors to receive medical, surgical and anesthesia services.
We recognize that a parent's presence is very comforting to a child. You may stay with your child until they are brought to the operating room. Parents and child are reunited in the recovery room as soon as possible after surgery.
